2018 Crafter 95k Miles - 4 injectors in 1k miles / 4 weeks - cause?

This 2018 Crafter with approx 95k miles has had 4 injectors fitted in the past 1000 miles / 4 weeks.

Van was bought privately a few months ago / a few thousand miles ago. It had last service done by VW franchise at 85K miles.

Yes 4 different installations - not good.

Before each injector was fitted it was clearly running on three cylinders. mechanic connected diagnostics and diagnosed faulty injectors. A different one each time. Immediately after the first 2 fitted (one at a time) it was running lumpy or still on three cylinders.

New fuel filter fitted when injector 1 fitted.

All injectors were reconditioned from a reputable supplier.

On to a different mechanic to fit injector 3 (he confirmed it was a different injector than 1 and 2) Ran smooth for a week, then 4th injector failed.

I have no issues with the garage that fitted the last 2, no reason to believe they have done anything wrong. They have VW experience and have been very helpful.

4th Injector (new Delphi, not reconditioned) to be fitted next Monday or Tuesday.

What possible causes could make these injectors fail?

Current mechanic suggests the possibility of bad fuel in the past, but this is a guess.

Concerned that some cause remains and the new injectors could fail!!

Some history:

Just before these injectors failed a good brand diesel fuel additive was added - I am doubtful that this could have caused an issue.

After injector 2 was installed the DPF regen light came on. A long drive did not resolve that.
After injector 3 was fitted the pressure levels on the DPF were measured and not a serious issue, however the DPF error could not be cleared and a forced regen could not be done.

A DPF "specialist" with programming experience checked the van and found that someone had partially disabled the the DPF system but the DPF had not been drilled.
The "specialist" fully disabled the DPF system (his words) but did not drill the DPF. He suggests if the van suddenly becomes underpowered he recommends drilling the DPF.

Could any DPF issues be a cause of the injector failures?
Whether yes or no to above - Should we have the DPF drilled.
